(1) Two Personal/Professional References (full names, phone numbers, and email addresses)
(2) Emergency Contact (full name and phone number)
(3) A copy of your government issued ID (driver's license, military ID or passport).
*JPEG, PNG or PDF preferred.
Submit your ID using the link below or email a copy to volunteer@sosc.org before or after you complete the application process.
