New Athlete Application

Athletes with intellectual disabilities, ages 8+

New athletes must complete and submit an Athlete Registration Form to get started. Once registered, SOSC staff will contact athletes to review available sports in their community and assist with season registration. From there, athletes will connect with their coaches and begin training!

Atletas Nuevos

Current Athlete Renewal

Athletes with intellectual disabilities, ages 8+
Athletes whose registration is set to expire before the end of their upcoming sports season will be notified two months before the season begins. To remain eligible, they must submit an Athlete Registration Renewal Form by the deadline, usually one month prior to the start of the season. Athletes whose medical clearance is set to expire before the end of their season must instead complete and submit a new Athlete Registration Form.

*DO NOT complete this form unless you have previously filled out the New Athlete form.
